GDPR Notice

This privacy notice has been prepared by Plaza Dental Oral & Dental Health Clinic ("the Clinic") pursuant to Article 10 of Turkey's Personal Data Protection Law No. 6698 ("KVKK"), to inform you about the processing of your personal data.

2. Personal Data We Process

The following personal data may be processed by the Clinic through appointment and contact forms on our website, phone/WhatsApp inquiries, and during examination and treatment processes:

- Identity data: First and last name, Turkish national ID number, date of birth

- Contact data: Phone, e-mail, address

- Health data (special category): Medical and dental history, statements of existing conditions and allergies, medications used, oral and dental examination/diagnosis/treatment records, radiological images (panoramic X-rays, etc.)

- Appointment data: Appointment date/time, preferred doctor and service

- Visual records: Intraoral photographs and images taken before/after treatment — with your explicit consent

- Financial data: Invoice and payment information

- Transaction security data: IP address, browser information, cookie data

3. Purposes of Processing

- Receiving, creating and managing appointment requests

- Providing oral and dental health examination, diagnosis and treatment services

- Planning, performing and following up treatments

- Communicating with the patient and addressing requests

- Sending appointment and check-up reminders and measuring patient satisfaction

- Carrying out invoicing and payment processes

- Fulfilling legal obligations (Ministry of Health regulations, tax legislation, etc.)

- Ensuring the security of the Clinic and patients, preventing misuse

- If you have given explicit consent, sending informational, campaign and reminder communications

4. Legal Basis for Processing

Your personal data is processed under one or more of the following legal bases set out in Articles 5 and 6 of the KVKK:

- Explicit provision under law (health and tax legislation)

- Direct relation to the establishment or performance of a contract

- Compliance with a legal obligation

- Necessity for the establishment, exercise or protection of a right

- Necessity for our legitimate interests, provided that your fundamental rights and freedoms are not harmed

- Processing of special category health data under Article 6 of the KVKK by persons or competent authorities subject to confidentiality obligations, for the purposes of protection of public health, preventive medicine, medical diagnosis, treatment and care services; outside this scope, based on your explicit consent

- Your explicit consent for visual records taken for treatment purposes and for marketing/informational communications

5. Recipients of Personal Data

Your personal data may be transferred to:

- Contracted dental prosthesis / dental laboratories (for prosthesis, implant superstructures, etc.)

- Imaging and laboratory service providers (when required)

- Private health insurance companies or contracted institutions, where treatment costs are covered through them (with your request/consent)

- The Ministry of Health and other competent public authorities (where legally required)

- Our accountants, auditors and legal advisors

- IT, software and hosting service providers (subject to confidentiality agreements)

6. Method of Collection

Your data is collected directly from you via appointment and contact forms on our website, phone calls, WhatsApp messages, e-mail, and visits to the Clinic, as well as during examination and treatment processes; where necessary, from healthcare institutions to which you are referred or from which you receive services, in electronic and physical form.

7. Your Rights (KVKK Article 11)

Pursuant to Article 11 of the Law, you have the right to:

- Learn whether your personal data is processed

- Request information about such processing

- Learn the purpose of processing and whether the data is used in accordance with that purpose

- Know the third parties to which the data is transferred, domestically or abroad

- Request correction if the data is incomplete or inaccurate

- Request erasure or destruction of the data under the conditions set out in Article 7 of the KVKK

- Request that the actions taken under the above be notified to third parties to whom the data has been transferred

- Object to any adverse outcome arising from the analysis of the data solely by automated systems

- Claim compensation for damages incurred due to unlawful processing of the data

8. How to Submit a Request

You may submit your requests under the KVKK to the Clinic via the following channels, together with documents verifying your identity:

Your request will be processed free of charge within 30 days at the latest.

9. Data Retention Period

Your personal and health data will be retained for the periods stipulated by applicable health and other legislation, and for at least the minimum periods required by law for medical records and patient files. At the end of these periods, your data will be deleted, destroyed or anonymized in accordance with applicable legislation.
